Patient Discharge Instructions for Coumadin

  • Upon discharge from the hospital, you will need to be on daily Coumadin dosages for three weeks from the date of your surgery (unless specified by your physician).
  • You will need to have your blood pro time levels drawn weekly or biweekly (as specified by your physician) to determine the appropriate Coumadin levels. These results will be sent to your orthopaedic surgeon or family physician the same day. You should expect a telephone call with these results on the same day or the day after. If you fail to receive a call, then your physician should be notified.
  • Your physician will inform you of Coumadin dosages based on your pro time results. Only take the dosage that your physician prescribes to you.
  • The recommended blood levels for Coumadin are determined by your pro times.
